Family Care Coordinator

Classification: Full-time

Working Hours:  Office hours per policy or hours dictated by workload and call schedule; Provides “24 hour on-call services” 7-9 days per month, including approximately one weekend per month 

QUALIFICATIONS

  • Licensed registered nurse or undergraduate degree in Human Services, Counseling, Psychology or Social Work with a minimum of two years’ experience preferred
  • Possess strong organization and interpersonal skills
  • Effective communication and presentation skills
  • Effective leadership skills demonstrated in clinical, educational, and administrative contexts
  • Ability to practice with a high degree of autonomy in a self-directed manner, demonstrating innovation and creativity
  • Ability to work collaboratively within an interdisciplinary team
  • Valid driver’s license, reliable automobile, and proof of automobile insurance

SUMMARY

This position is responsible for creating a positive environment and heightened awareness of donation at a designated hospital in the MSTF service area.  This position is dedicated to achieving the primary objectives of (1) maximizing organ donation (2) providing the utmost care and support to potential donor families and (3) fostering a learning atmosphere through education and communication of donation information.

RESPONSIBILITIES

  • Functions as an expert resource within the hospital for organ donation
  • Develops and maintains effective working relationships with key hospital staff 
  • Maintains high visibility within the hospital by conducting daily rounds in designated areas
  • Collects and reports donation outcome data to various levels of clinical and administrative hospital staff, as well as the Manager of Hospital Services
  • Collaborates and provides assistance to hospital leadership to ensure compliance with  JCAHO standard PI.1.10 and LD.3.110 and CMS Conditions of Participation regulation
  • Schedules, develops, provides, and evaluates training and educational programs on all aspects of the donation process for hospital staff
  • Responds appropriately to or designates an appropriate individual to respond to potential organ donor referrals
